Referral Procedure:
The participant schedules an initial enrollment to determine the most appropriate group placement. Completed Referral Information Sheet must be received one week in advance of date participant is scheduled for enrollment.
Referral Information Sheet Guidelines:
Please include the following items:
A. Official description of the incident(s) that led to the referral, such as a copy of the police report, victim affidavit, or PSI
B. Any prior known history of intimate partner violence
C. Current and prior legal charges, including drug or alcohol-related as well as violent charges other than DV
D. Information regarding protection or restraining order currently in place.

Admission/Exclusion Criteria:
- Males at least 18 years old.
- Have a documented history of intimate partner domestic violence as corroborated by referral source.
- Able to participate in the assessment/intake process to the degree needed to enroll in services.
- Must be medically stable.
- Must commit to being drug free while receiving services.
- Must not have severe mental health diagnoses and/or acute psychiatric symptoms that would preclude the ability to engage in and benefit from Transform group services. Severe mental health diagnoses may include:
- Active psychosis, delusions, hallucinations, suicidal and or homicidal ideation; not properly oriented according to standard mental status examination, etc.
